1. Why we collect your information
We collect personal and health information to:

provide safe and appropriate massage treatment;

manage appointments, reminders and payments;

comply with legal, regulatory or insurance requirements; and communicate with you about your care (including recalls, follow-ups and invoicing).
We only collect information that is reasonably necessary for these purposes.
2. What we collect
Typical information collected includes: name, date of birth, contact details, emergency contact, relevant medical history, medications, treatment notes, and payment details. You are not required to provide information you do not wish to, but withholding relevant medical information may affect the care we can safely provide.

3. How we collect information
We collect information directly from you (intake forms, consultations, phone/email), and where necessary from third parties with your consent (e.g., referring practitioner). We do not collect more sensitive information than is needed.

4. How we store and protect your information
Your records are stored electronically in Halaxy. We take reasonable steps to protect your information, including:

encryption of records at rest and in transit;

using secure, password-protected systems with multi-factor authentication;

restricting access to authorised personnel only;

secure encrypted backups; and
-
staff privacy training and confidentiality obligations.
If we use external providers (booking software, SMS reminders, cloud backup), we ensure contracts include privacy and security obligations. If you would like details of our technical measures, please contact us.
5. How long we keep your information
We retain clinical records in line with professional guidance: a minimum of 7 years from last entry for adults; for clients who were minors at the time of care we retain records until they reach 25 years of age (or longer where required by law). If you would like us to destroy or de-identify your records earlier, contact us and we will consider the request in line with legal and professional obligations.
6. Use and disclosure of information
We use your information for the reasons listed above. We may disclose information to: other health professionals involved in your care (with your consent), billing or legal advisers when necessary, and third-party service providers who assist with appointment management, billing or IT (under contract). We will not sell your information. Where disclosure involves transfer overseas, we will only do so if the recipient provides appropriate protections or with your consent.

7. Your rights — access, correction and complaints
You may request access to or correction of your personal and health information. To request access or correction, contact resolve.massage@bigpond.com. If you have privacy concerns, please raise them with us first. If not satisfied, you may contact the Office of the Australian Information Commissioner (OAIC).
8. Data breaches and notification
If a data breach occurs that is likely to result in serious harm, we will follow the Notifiable Data Breaches process and notify affected individuals and the OAIC as required by law. For more information about the NDB scheme, contact the OAIC.
9. Consent
By providing your information and booking treatment with Resolve Massage Therapy, you consent to the collection, storage and use of your personal and health information as described in this policy. You may withdraw consent to future uses at any time by contacting us, however withdrawal of consent may limit our ability to provide services.
10. Changes to this policy
We may update this policy from time to time.
